The full account of first-time title defenses:

Steinitz: won title vs. Zukertort, 1886. Retained vs. Chigorin,
1889.

Lasker: won title from. Steinitz, 1894. Retained vs. Steinitz, 1897.

Capablanca: won title from Lasker, 1921. Lost to Alekhine, 1927.
Technically, one might argue that since Lasker resigned his title _to_
Capablanca 1920, without playing, that Capa was defending the title in
1921. Lasker did in fact insist that in the formal terms of the match,
Capa was designated champion and he the challenger.

Alekhine: won title from Capablanca, 1927. Retained vs. Bogolyubov,
1929.

Euwe: won title from Alekhine, 1935. Lost to Alekhine, 1937.
Alekhine never played another title match after that, and died in
1946.

Botvinnik: won vacant title, 1948. Retained vs. Bronstein, 1951.

Smyslov: won title from Botvinnik, 1957. Lost to Botvinnik, 1958.

Tal: won title from Botvinnik, 1960. Lost to Botvinnik, 1961.

Petrosian: won title from Botvinnik, 1963. Retained vs. Spassky,
1966.

Spassky: won title from Petrosian, 1969. Lost to Fischer, 1972.

Karpov: Declared champion after Fischer's abdication, 1975. Retained
vs. Korchnoi, 1978.

Kasparov: won title from Karpov, 1985. Retained vs. Karpov, 1986.

Kramnik: won title from Kasparov, 2000. Retained vs. Leko, 2004.

I have included only the "classical" title, which with the exception
of 1948 has always been decided by match play, not the FIDE titles won
under various formats 1993-present.

And Fischer lost it without defending at all.
