Is this a proven draw if the pawns are two files apart? What about
other distances apart?

Very often just K+B+P vs K+B is a win. Benko demonstrated that in a study with
the pawn still on its start position.

In Basic Chess Endings, he says, "Bishop plus two pawns win against Bishop as a
rule without any trouble."

I would expect that the main type of draw, as I have seen shown in most endgame
books, would be when the bishop can sacrifice itself for one pawn and the other
is a rook pawn of the wrong color for the bishop, leading to that well-known
draw.

Those are opposite colored bishops, which is quite different than same color
bishop endings.

In endings with Bishop and two isolated pawns against Bishop *of opposite
color*, the general rule is that, if there are two or more files between the
pawns, they will usually win, but if there is only one file between the
pawns it's usually a draw.
